A postmortem is how a team learns from an incident without assigning blame. It reconstructs the timeline, finds the root cause, and commits to action items so the same failure does not recur.
This template is the built-in Incident Postmortem starter project in Atlas. It ships with milestones for drafting the timeline within a day of resolution, identifying the root cause, running a blameless review, and shipping the action items.
Because it is a live Atlas project, the action items are tracked tasks with owners rather than lines in a document, so the follow-through actually happens.
